What is the role of the Discharge Planning staff?

  • Assist with complex discharges
  • Help patients make a smooth, timely and safe transition from hospital to home

Post Acute Care (PAC):
What does the PAC Program do?

  • Provides community-based services to assist people to recuperate after leaving hospital
  • Aims to prevent hospital readmission

What services does the PAC Program provide?
A range of community-based services based on the person?s? individually assessed needs.

The most common services provided are:-

  • District Nursing??
  • Personal Care
  • Home Care Services

Services are arranged by PAC staff and provided for the duration of the recuperative period and are generally of a short-term nature

Hospital in the Home (HITH):
What is Hospital in the Home?
HITH is the provision of acute care in the home. Patients are classified as in-patients for the duration of the HITH episode.
Assessment is essential to establish suitability prior to commencing on the program

Post Acute Care:

  • Shire of Southern Grampians
  • Also clients residing in the area serviced by the Casterton District Nursing Service.

Hospital in the Home:
The city of Hamilton and an 11km radius
The township of Coleraine including the area serviced by the Coleraine District Nursing Service.
Special circumstances for the outlying areas of Dunkeld, Penshurst and Cavendish
